E.S.A 50 FORM Options

hi all, has anyone had any experience of filling in the E.S.A 50 form? i have been sent one to fill in and it looks so hard. Why does getting any help mean taking yourself to pieces everytime? it is so hard to be positive when you have to keep talking about all of your worst points and struggles. The c.a.b have said that they will help me fill the form in,is this a good idea? any advice would be great. thankyou.x

Me? I'd take the cab help, as the are seeing far more of these forms. I haven't seen one yet, but I hate filling in forms. I do agree that is so depressing to concentrate on what I can't do. Good luck with it all.

Hi Kerrie ,
It is worth a try to get CAB to help you with this , however not all CAB's can offer this service .
I wrote a basiic guide in September and here it is copied and pasted...

The ESA50 Form

The ESA 50 form has a time limit for you to return it, and it is important that you complete and return it to get to the address within 4 weeks of receiving it.

It is used by the DWP as part of the process for working out your eligibility for Employment and Support Allowance. It is important that this is completed as fully and accurately as you can.

Unless you are in one of the exempt groups, It will be used at your medical (Work Capability Assessment) and will also be used by the DWP Decision Maker to assess your claim.
It will also be used if you have to appeal a decision at a later stage.
Read all the paperwork that you received with the form thoroughly.

It is better that you fill out the form as only you know the full extent of your illness, and how it affects you. It may be useful to have friends or family there to remind you of any issues you miss. They can also help you fill out the form.
If someone fills out the form for you get them to fill in the relevant box.

Do not underestimate how your illness affects you. But also do not exaggerate. Give real-life examples of how your condition affects you. There are boxes on the form that let you do this. Try and use all the space you can with relevant detail.

Make a rough draft of your answers, before filling out the form, and make a photocopy of the form before you send it back, if possible.
If you can, send the form back by recorded delivery.

Keep a diary if you can of how your condition affects you on a daily basis and also try to obtain reports or letters from your GP or consultant to support your claim.

Send this extra evidence to the address at the top of your award letter from Job Centre Plus. Ensure that you write your name, date of birth, and National Insurance number on each loose sheet you send.

In the meantime, keep on sending in medical certificates from your GP or Consultant until you are told in writing that you no longer have to.Try and keep copies of them yourself.




Even if you honestly answer that you can do one of the descriptors (walking, sitting etc) Then state in the box below it any difficulties you do have with real life examples . As stated above it is often good to have friends or family there to remind you of occasions when you have struggled.
Just work through the form a question at a time .. here is a link to a copy of the form and it might be worth filling this out as a draft ESA 50 form

Best advice is to try and ensure as many of the white boxes are as full of relevant information as possible .

Hi Rich, Sorry to hear that you are housebound at the min, i know what that is like as i am incapable of going out alone. Thankyou for your advice i am going to ask the c.a.b to help me. I have now also got a letter fom my g.p supporting me in that i do not leave the house alone due to panic attacks and anxiety and i am praying that it helps. THANKS AGAIN. Kerrie.
